The Heart of the Mountains Hospice, Inc. strives to create options with dignity and integrity in providing high quality home hospice and respite care for individuals and their support systems within the communities of Grand County.

Our primary goal is to uphold the Heart of the Mountains Hospice, Inc. mission of providing in-home supportive care to the terminally ill.

The date for the 10th ANNUAL GRAND SWING to benefit HEART of the MOUNTAINS HOSPICE presented by FOWLER & PETH has been set!  It will be held on July 17, 2012 at Pole Creek Golf Course with a 9:00 a.m. shotgun start.  So mark your calendars now!  And keep your eyes peeled for some special TENTH ANNIVERSARY celebrations!
