The Heart of the Mountains Hospice, Inc. hopes that you will find the following sources of information to be helpful. Please come check out our classes, read the available books, and search the internet to learn more about the wonderful services that hospice provides.

GRAND COUNTY RESOURCES

The Grand County Council on Aging is a 501(c)(3) non-profit organization that assists senior and disabled residents of Grand County to maintain independence. They are located at 469 E. Topaz Ave. in Granby, CO. Their phone number is (970) 887-3222. In addition to on-going events for seniors (for example bingo and exercise classes), they offer nutrition and meal sites, provide a Senior Van for "necessary" trips when a senior is unable to drive, and Ensure at cost. No one is turned away from transportation or nutrition needs due to lack of funds. Cliffview Assisted Living Center is located at 710 Eagle Ave., Kremmling, CO. This facility is vital to Grand County residents in providing assisted living. For more information, they can be reached at (970) 724-3530.

Long Term Care at Kremmling Memorial Hospital - Call (970) 724-3442 and request to be connected to the Social Worker.

Bereavement Support Group - Maverick's Grille, Big Shooter's Coffee Shop, and the Hospice Staff host casual coffee gatherings. The meetings take place in Granby (Maverick Grille) on the 2nd Wednesday of the month and in Kremmling (Big Shooter's) on the 4th Wednesday. The purpose of the group is to provide support and education to people that are grieving the loss of a loved one.

Care Giver's Support Group -  This group meets on the 2nd Thursday of the month from 2:00 p.m. to 3:00 p.m. in the Granby Senior Center Building located at 125 N 3rd Street, Granby, CO and on the 2nd Friday of the month from 10:00 a.m. to 11:00 a.m. at the Silver Spruce Senior Apts. located at 106 6th Street, Kremmling, CO.

• The Colorado Center for Hospice and Palliative Care work to increase access to palliative and hospice services and promote quality standards of care for all Coloradans.

• The National Hospice and Palliative Care Organization is the oldest and largest nonprofit public benefit organization devoted exclusively to hospice care.

• This Hospice and Home Care site is a great portal to information about resources for life-threatening illness and end of life care.

BOOKS - The Heart of the Mountains Hospice, Inc. is fortunate to have a large collection of books housed at the Granby Library in Granby, Colorado.
